— Brian David Cook, 42, Kokomo, passed away at 1:39 p.m. Friday, Jan. 4, 2013, at his residence. Brian was born Dec. 20, 1970, in Kokomo.

He had worked at Tyson Foods.

Survivors include his daughter, Amber Cook, Plato, Mo.; step-father and his mother, William J. and Barbara K. Haley; and three brothers, Nick Cook, Scott Cook and Dustin Phillips.

Funeral services will be 11 a.m. Friday at Ellers Mortuary Webster Street Chapel, 3400 S. Webster St., with Donnie Hughs officiating. Visitation will be from 10 to 11 a.m. Friday at the mortuary. Burial will follow in Greenlawn Cemetery in Greentown.
